| Appointment Policy |

- Make your appointments at times that you can reasonably expect to keep them.
- It is important that you notify us at least 48 hours before your appointment if you need to cancel or reschedule because we block out a specific time period to meet with you.
- If you do not cancel your appointment within 24 hours or if you miss an appointment, a $25 fee may be charged.
- If an emergency does arise, we ask that you give us as much notice as possible so that the valuable appointment time may be given to another client in need of our services.
- Clients coming late for their appointment will be seen for the remainder of the treatment or counseling session.

| All services fees are due upon completion of your office visit. American Express, Discover, Mastercard, Visa and Cash are accepted for your payment of services. |

Please call the office, so we can verify your insurance coverage.
Health Insurance is accepted for the following payors:

Beechstreet PPO (Aetna, Cigna, UnitedHealth Care)

Private Healthcare Systems (PHCS)
For reimbursement from your health insurance payor:

Get a referral from your primary care physician

Get a prescription for medical nutrition therapy

Obtain a claim form from insurance payor
We will you provide with all of the required insurance codes. |
| Flexible Spending Account (FSA) |

Establishing a FSA is another financial option for taking care of your preventive health needs. A FSA lowers your taxable income. Contact your Human Resource Specialist to get an account set-up. All services are eligible for reimbursement from your FSA. |
